Some Basic Background Information

Cold sores are the external symptoms of an active oral herpes outbreak (aka "herpes labialis") that appear most typically on the lip and/or around the mouth--they are very rarely found *in* the mouth, and if this is the case it's much more likely that you've got something known as a canker sore which is caused by a bacteria and completely unrelated to cold sores and the herpes simplex virus.

Herpes is transmitted from one person to another through contact with open sores or bodily fluids--the most common way that people get oral herpes is by being kissed by a close relative when they are a small child who has an active outbreak or, possibly, is experiencing something called asymptomatic viral shedding (this is where you're still shedding the virus and are contagious despite the fact that you don't have any visible symptoms such as sores or lesions on the skin).

There is no cure for herpes, and once you got it you've got it for life--the herpes virus spends the majority of its time in what's called the "remission phase" where it remains dormant in the sensory nerve cells of your face near the site of initial infection, sometimes for years at a time between active outbreaks--just because you haven't had an outbreak for several years does NOT mean that you don't have the herpes virus anymore and can' t ever get another outbreak. Even if you haven't experienced a cold sore in 20 years, if you ever had one that means you're infected and the virus is still there and could potentially emerge at any moment.

Fast and Simple Treatments

There's plenty of prescription and OTC (over-the-counter) treatments such as anti-viral pills like Acyclovir and OTC creams like Abreva that you can check into if you so desire, but this article is about home remedies for cold sores that work, so let's dive in.

Over the years I've tried a great many various treatments, and most of them are simply anecdotal wives tales that simply don't work or work to a very negligible degree, a few are consistently pretty effective, and from that I've come up with a little system that I've found to be very effective (do each of these steps once every 2 hours as soon as you feel the tingling of an oncoming cold sore):

1. Keep the cold sore clean and dry by washing it with a damp wash cloth and soap--use a new wash cloth each time and toss the old one in the laundry after you use it to keep from re-infecting yourself. It would also be a good idea to replace your toothbrush with a new one and then again after the cold sore has gone (I actually rinse mine in rubbing alcohol each time after I use it if I've got an outbreak to disinfect it).

2. Sterilize a needle with rubbing alcohol or boiling water and use it to puncture the cold sore(s), then squeeze and drain every last drop of clear/yellow fluid from them (dab with a kleenex to soak it up).

3. Apply a q-tip with benzyl alcohol ("Zilactin") or, alternatively, isopropyl alcohol (aka rubbing alcohol), but use benzyl alcohol if you can, to the affected area. 4. Apply clearasil or nail polish remover to the area with a q-tip--this will keep it completely dry and soak up any fluid for the next 2 hours until you repeat this process.

This is a VERY effective little procedure and, in my experience, will have that sucker GONE within 24-36 hours if you're on top of things and stay on schedule with the treatment. It may also help to take some Vitamin C supplements to boost your immune system along with some l-lysine which has been shown in multiple scientific studies to interfere with the virus' replication thereby shortening the duration and severity of a cold sore outbreak. What Is This Thing On My Lip?

If you have a cold sore on your lip then what's happened is that the herpes virus that was dormant in the nerve cells around your face has, for some reason (something probably caused your immune system defenses to drop for a short period, thereby allowing the virus to go active) decided it was going to come out and play--fun for it, no fun for you, right? Oral herpes (aka HSV, or herpes simplex virus) is the most common form of herpes and is medically known as "herpes labialis", which is the infection that occurs when the virus comes into contact with oral mucosa or abraded skin.

Other disorders such as herpetic whitlow, herpes gladiatorum, ocular herpes (keratitis), cerebral herpes infection encephalitis, Mollaret's meningitis, neonatal herpes, and possibly Bell's palsy are all caused by herpes simplex viruses.

The cold sore on your lip, as you probably know, will not be there forever, but while it is there it's painful, annoying, itchy, and worst of all: embarrassing! For many people, cold sores are so unsightly and embarrassing that they won't even leave the house until they're completely cleared up! Regrettably, no cure or vaccine is currently available, although vaccines of varying effectiveness are currently in phase III clinical trials with the NIH (National Institute of Health--search Google for "herpevac" and "zostavax").

Available Treatments

Mind you, no matter what treatment you take, there is currently no "cure" for cold sores, and once you've got it, then it's with you for life. Herpes cycles through dormant and active cycles, being dormant the great majority of the time, and only causes active outbreaks a few times a year that will typically last from between 2 and 21 days, often right around a week for most people.

Prescription anti-viral medications for herpes work by interfering with viral replication, thereby slowing the replication rate of the virus and giving the immune system more of a chance to shut the virus down before it can cause problems. Acyclovir was the original anti-viral prescribed for HSV, and is currently the recommended drug for suppression of HSV during the last months of pregnancy to help prevent neonatal herpes. The other two prescription anti-virals typically prescribed for cold sores are Famcyclovir and Valacyclovir. All three of these medications will essentially result in the same thing for the average cold sore sufferer: they will reduce the pain and irritation experienced by the individual, and they will shorten the duration of the outbreak by 1 to 2 days, that's it.

I personally recommend that anti-virals, if you have a prescription for them, are combined with OTC (over-the-counter) and some simple home treatments that, when used together, can significantly shorten the duration of the outbreak down to as little as 24-48 hours in total length.

Cold Sore First Aid

Ok, here's what to do:

1. As soon as you feel the tingling of an oncoming cold sore, immediately start taking your anti-viral medication if you have a prescription.

2. Keep the area clean by washing it once every 2 hours with a damp wash cloth and soap--use a new wash cloth each time so as not to spread the virus to any other areas of your body (you don't want it in your eyes--herpes keratitis is a leading cause of blindness in the United States).

3. Ice the area with an ice cube for approximately 10 minutes at a time every 2 hours.

4. Apply benzyl alcohol with a q-tip to the area after you ice it. Let it absorb and completely evaporate. Isopropyl alcohol (plain rubbing alcohol) can be substituted if you can't get any benzyl alcohol.

5. Apply some 10% benzyl peroxide acne medicine (like Clearasil) to the area--this will soak up any and all excreted fluid between now and the next time you wash it in 2 more hours.

This procedure will drastically reduce the length of your outbreak and significantly reduce the severity of the cold sore as well. If you do this early enough before the cold sore has even emerged when you're feeling prodromal symptoms (symptoms that occur prior to an outbreak, the most common of which is tingling), then oftentimes the cold sore will be prevented and never emerge in the first place.

If you start this procedure once you've already got a cold sore I would advise you to ensure that you drain every last drop of fluid from it before washing it in step 2--treat it like you would a pimple, just pierce it with a sterilized (use alcohol to do this) pin and then squeeze and drain the liquid onto a kleenex that you can blot it with.

Sending care packages to our troops stationed overseas is a great way to show your support for their efforts. However, you must plan the contents of the package carefully to ensure that the soldier will receive everything in useful condition.

Every member of the armed services would love to have homemade cookies and other goodies to enjoy and share with others. Such items are a thoughtful gesture, but can raise some problems which may keep them from reaching troops. Each country in which our troops are stationed have different customs rules on what can enter the county including food products. Often homemade goodies do not have an ingredient list and can be removed for that reason. Commercially packed goodies have been authorized and will not be removed from packages. In addition, remember that the weather will be very different from ours – for example, temperatures in the Iraq/Afghanistan area can reach over 110 in the summer. Any products sent during the summer must be able to withstand such temperatures.

Keeping the package size manageable is another item to plan properly. The United States Post Office offers two boxes which make ideal sized care packages – the flat rate “shoe-box” size, and the flat rate 11 7/8” by 3 3/8” by 13 5/8” box. The boxes are priority shipped to the APO address for the flat rate of .10 for each box. There is also a flat rate envelope available – please see http://www.usps.com or your local post office for information. In addition, keeping the box weight to about 6 pounds makes them very easy to handle. If you do not have a specific person to send a care package, you can use your web browser to find programs that assist in matching troops to givers. One such program can be found at http://www.anysoldier.com and often lists specific requests from soldiers or squads. There are currently over 3,000 names on their contact list.

Remember when sending packages to military members that the United States Postal Service delivers the packages to the APO address. After that, delivery is dependent upon the military mail services and may take longer than what we normally expect. In addition, you will be required to complete a customs certificate listing the items within the package and their cost. These forms can be completed and printed at http://www.usps.com or obtained from your local post office.

Creating a care package can be quite easy, whether you do it yourself or use one of the many services that are available. If you wish to purchase a care package, and there are many types available from entertainment to hygiene to goodies, you can find a company by typing in “soldier care packages” into your web browser. If you wish to stock one yourself, please remember these guidelines:

Obscene articles (prints, paintings, cards, books, videos)

Pornographic or sexual items

Anything with nude or seminude persons

Non-authorized political materials
Religious materials contrary to the Islamic faith(personal items are allowed)

Pork or pork by-products

People often ask if phone cards are a good idea for a care package. There are some issues with the phone cards primarily because you need to have specific cards. It is wise to only send cards if a person has requested them because you know they will be used.
